normal align=left style='margin-right:-1.0pt;text-align:left; text-indent:15.75pt;line-height:150%;text-autospace:none'>2.4建设规模：本工程建筑面积46562.21平方米。主要为施工图示范围的南区门诊医技大楼及第一住院楼（含B1、B2）室内外改造、B3外立面改造、结构加固、总平工程，新增门诊及第一住院楼、第二及第三住院楼物流系统，第一住院楼医气工程，风雨连廊、屋面改造、屋面排烟机房及南区、北区门禁系统改造等内容；保供相关工作内容。估算投资额约20799.97万元。

normal style='text-indent:15.75pt;line-height:150%'>3.1.1须具有独立法人资格

normal style='text-indent:15.75pt;line-height:150%'>3.1.2 须具有建设行政主管部门颁发的房屋建筑工程监理甲级及以上资质的单位；；

normal style='text-indent:15.75pt;line-height:150%'>3.1.3信誉要求：不存在投标人须知第1.4.3项规定的限制投标的情形；

normal style='text-indent:15.75pt;line-height:150%'>3.1.4企业注册地不在四川省行政区域内的省外企业须提供在有效期内的四川省住房和城乡建设厅官网已公开的入川信息网页截图（如投标人为联合体，则联合体所有成员均应具有）。

normal style='margin-left:5.25pt;text-indent:10.5pt;line-height: 150%'>3.1.5所有投标人应具有针对本标段的《成都市建设领域市场主体信用记录（评价）网络登记表》、《成都市建设领域从业人员信用记录（评价）网络登记表》；

normal style='text-indent:15.75pt;line-height:150%'>3.1.6拟派项目总监理工程师：具有全国注册监理工程师（专业：房屋建筑工程）执业证书，业绩要求：无（业绩要求），须为本单位人员(若为联合体投标，须为联合体牵头人人员)。

normal style='text-indent:15.75pt;line-height:150%'>近年（2020年01月01日至投标截止时间，不少于3年）已完成的项目共不少于1个类似工程业绩。

normal style='text-indent:15.75pt;line-height:150%'>类似工程业绩是指:单个合同项目投资额不低于15000万元的公共建筑工程监理业绩；

normal style='text-indent:15.75pt;line-height:150%'>3.2本次招标不接受联合体投标。

normal style='text-indent:15.75pt;line-height:150%'>3.3 各投标人均可就上述1个标段投标，可中标的标段数量为1个。

normal style='text-indent:15.75pt;line-height:150%'>3.4入围条件：房建监理投标人开标当日在“成都市建筑市场信用信息管理系统”中被列入市场禁入名单的（联合体投标的，组成联合体的各投标人任意一家被列入市场禁入名单的），在交易云平台勾选主体信用评价的专业类别与其投标使用的资质类别不一致的，一律不能成为合格投标人。
